Agreement & Disclaimer Form for Epic Leader Retreat Participants

By signing this Agreement & Disclaimer form, I, the undersigned participant, agree to the following terms and conditions during my stay at the Epic Leader Retreat:

1. I agree to adhere to the retreat schedule, guidelines, and rules provided by the retreat organizers.

2. I understand that the retreat involves physical activities, and I confirm that I am physically capable of participating in these activities. I acknowledge that my participation is at my own risk and will not hold the organizers liable for any injuries or accidents incurred.

3. I understand that photographs, audio, and video recordings may be taken during the retreat. Any such materials I capture are intended solely for my personal use and may not be used for commercial purposes or distributed publicly without the written consent of the Epic Leader Retreat organizers and any individuals featured in the content.

4. I acknowledge that all content presented during the retreat is the property of the Epic Leader Retreat and may not be reproduced or distributed without written permission.

5. I agree to respect the privacy and personal boundaries of other participants, staff, and the retreat venue.

6. I acknowledge that any damages caused by me to the retreat venue or equipment will be my financial responsibility.

7. I confirm that I have read and understood the retreat's cancellation and refund policies.

8. I understand that the organizers reserve the right to dismiss any participant for inappropriate or disruptive behavior without refund.

9. I confirm that I hold valid and comprehensive travel insurance coverage, including medical and liability insurance, which is fully valid and applicable during the retreat period and throughout my stay abroad. I acknowledge that I am responsible for any medical expenses or liabilities that may arise during my participation in the Epic Leader Retreat, and I will not hold the organizers responsible for any such expenses or liabilities.

10. I confirm that I have fully understood the program details, objectives, and requirements of the Epic Leader Retreat. I commit to participating fully and actively in all retreat activities, maintaining a respectful, open-minded, and cooperative attitude throughout the program. I will strive to make the most of this transformative experience and contribute positively to the retreat community.

11. I agree that in the event I encounter any issues or concerns during the retreat that may negatively impact my experience, I will promptly and directly communicate with Ni Wa, the retreat organizer, to address and resolve the matter to the best of her abilities. I understand that open and timely communication is essential for ensuring a positive and fulfilling retreat experience for all participants.

12. I understand the importance of disclosing * any relevant medical information, including my medical history, medications, allergies, or other health concerns, to the retreat organizers prior to the start of the retreat ( enclosure 1) This information is essential for ensuring that appropriate precautions are taken, and any potential health risks are addressed or mitigated. I agree to provide accurate and complete medical information to the retreat organizers to ensure my safety and well-being, as well as the safety and well-being of other participants, during the Epic Leader Retreat.

By signing below, I confirm that I have read, understood, and agreed to the terms outlined in this Agreement & Disclaimer form.
